Blind residents smoking, dirty facility, no meds: TN nursing home facility loses license

A Middle Tennessee assisted living facility has had it's license suspended following a multitude of allegations which includes allowing a blind resident and others to smoke indoors, not providing proper care, and giving residents the door codes so they can exit the facility.
