NASHVILLE, Tenn.--The Metro Public Health Department has announced a potential privacy issue involving the personal information of Children's Special Services clients.

The department says index cards which contain CSS patient's names, social security numbers, addresses and medical code numbers went missing on July 18th.

The department doesn't believe the cards were stolen or taken by an unauthorized individual but instead possibly thrown away in a landfill. Clients will be receiving notification letters and those affected will have access to an identification protection service at no charge for 12 months.

Anyone with questions is asked to call 615-340-5377.
